One of the hospitals at the centre of a government-ordered maternity review has come under further scrutiny over the way it scanned pregnant women.
Channel 4 News has learned that a separate investigation is now underway into Oxford University Hospitals Trust following reports by this programme into the deaths and harms to babies born there.
One couple had complained that they were refused a scan and by the time their baby was born she had died.
